Name of School/group

Person in charge and  contact details

Grade and objectives

Expected number of participants and staff/leaders

Preferred dates

Arrival and departure times on first and last days

Will we run a full program or just partial? (please give details)

Will we cater all meals?

Are there any major special dietary requirements? (Halaal etc.)
